REFLECTOR: Moral Dilemma - POH

IIRC, John Denver went down in an early flight for him in the plane, not an
early flight of the plane.  He had recently purchased a flying plane.
Again, IIRC, fuel was low in one tank, the fuel valve/switch was located in
an odd position over his shoulder and it was known to stick (needed a vise
grip and a local A&P's assistance to turn it prior to this flight).  He took
off, and according to investigators, ran out of fuel in one tank, as he was
reaching over his shoulder to try to switch tanks, he banked into the ocean.
Again, if I recall correctly.
