REFLECTOR: PROPER ENDORSEMENT

Only for the annual conditional inspection. It is a standard format, but I  
don't have the exact one at the house. The rest, just enter what  you do and 
your certificate number and sign it. If you make a major modification  requiring 
further testing, I suspect that there is again a standard entry.
