REFLECTOR: Reflector Digest, Vol 102, Issue 82

When I worked for the FAA, we would sometimes be assigned to accident 
investigation to do the drudgery work for the real investigators.

I was always amazed that in almost all instances it was a series or 
chain of mistakes by the pilot/copilot/mechanic that led to the 
accident, Never just one. Break the chain and no accident.

Be vigilant out there!
