Andy is right. Some sort of lube in the process gives better results & makes it easier to do. I use Vaseline.

This looks like a dry flare.  Are you using any type of lubricant on the tool?  Just a drop of oil, cutting fluid, etc. will go a long way toward making a smooth face.

Thanks for the feedback.  The tool I have is a 37 degree tool.   I do not like the way it looks on the flare face. 
May be due to the tool's old age...  Attached is another pic, of the face.

The critical part is the face of the flare, which you can't see in the picture. It needs to be very smooth.  If thats ok, and the sleeve slides over the outside without binding it should be alright (as best I can tell from the photo). 
 
I never cared for that tool myself. A good choice that's not too expensive is http://www.aircraftspruce.com/catalog/topages/flaringtools2.php.
