REFLECTOR: Dear Unbelievable

Sorry but I agree with Bill.  Where was the on board fire extinguisher? 

Most of our accidents/incidents are the direct result of some sequence
of events that can include a duh!

We don't know all that went into the pre-flight arrangements and I'm
sure Dave, did a professional job, but obviously the extinguisher wasn't
on the check list.
