The transplanting surgeon is usually the procuring doctor as well - or part of the team - for liver and heart.  They don't want to be slowed down or delayed by weather. Speed is important, not only because of the organ - but because the surgeon's time is valuable - because they may be the transplanting surgeon upon return, that can be a long surgery after they have been in the air.

Kidneys fly commercial unaccompanied - because 'anybody' can remove a kidney. :o)  Of course, the OPO is picking up the tab...so they want to keep costs low.

The most common use would be for banked blood. I'm sorry, but Organ
Procurement Organization's dont' use angelflights. They use learjets..
And for good reasons. Lears are fast. Even though kidneys are good for
days... hearts.. 4-6 hours I believe..
