REFLECTOR:Alex...see attached... Axle locking

You cannot just screw in the axle and screw down the set
screw since the screw may not go down into the grove so
essentially the axle is not locked.

You must rotate the axle in and out while tightening the
set screw until you feel it go down into the grove.  The
axle is then locked and the set screw is in shear mode if
the axle tries to rotate.  Since the setscrew is recessed
into the fork you can add another set screw on top with a
drop of locktite.  It will not come out for sure then.

Note that the axle may protrude slightly on other side when
the setscrew is properly in the grove.
