Jack,

Interesting we had the exact same failure.  set screw loosened, allowing 
the axle to unscrew enough to get hung up on the top of the guide bar.

Are we the only ones?

I'm also thinking about drilling on through the yoke(my new one) where the 
set screw hole is, drill through the axle in the same place, and insert a 
pin.    I'm somewhat worried about weakening the yoke with two holes?

Anbody else tackled this opportunity for catastrophe?   I think its an 
obvious design flaw that should be rectified.
