I just took a look at my aileron torque tube bearings/brackets. (It was
easy sice my plane hasn't flown yet and they were just laying on the
shelf.) One of the screws on the co-pilot bracket was pulled past the
bearing.  The screw head did not overlap the bearing enough causing the
thin outside edge of the head to bend back and pull past the bearing when
it was tightened.
